📝The Green Sleeve serves as a “medical passport” for patients. It contains documents that outline their healthcare wishes and goals. It ensures that all healthcare providers, regardless of setting, have access to a patient’s goals of care and interventional guidelines. Join us and get a better understanding.
📝This workshop is FREE, but they ask you register to give an idea how many will be attending. It is open to everyone; seniors, those with aging parents, or those that want to learn more.
